Does anyone know how to get it? I've beaten the game on normal. And i have played all of the missions from getting the clues? am i missing something? or is it in a special location? please tell!

The problem your having might be because you haven't played the whole game through without replaying missions. To get the achievement you need to play the whole campaign without replaying any missions and not losing any of your game progress by adding skulls or changing difficulty.
