when you turn on the second controller it will say push A to join if its through xbox live invite them to a Game....second player must create a Gamer profile on your xbox in order to join just make a fake one and delete it later

it is probably your router if you are playing online, make sure your nat is open, or it won't work most of the time, also your disk might be scratched if that doesn't work so you should return it for a new on.
