Did you start from the beginning of the mission or a rally point? It won't appear unless you start it from the beginning of the mission. Also, make sure your not in easy mode, you need to be in normal or harder. Other than that, it might just be unluckiness, sometimes it just doesn't appear for some reason, not sure why, but it usually is there the next time if this happens
